ICA Membership Application Form

Candidates for membership must fill in a Membership Application Form and provide the ICA with the following documents/information:

• Statutes / Rules / Bylaws of your organisation
• Annual Report / Audited Financial Statements
• Certificate of Registration

The completed form and supporting documents should be sent by email to hacquard@ica.coop.
